CCH is a government-approved Home Care Package provider offering consumer-directed care. We partner with recipients to design tailored care plans that support their unique needs, independence, dignity, and overall well-being, all in the comfort of their own home.

How It Works
Customised Care Plans
We work with our clients to create a care plan addressing their goals, needs and preferences, including services from approved third-party providers (separate to CCH) if required.
Flexible Care Levels
We cater to all Home Care Package Levels (Levels 1 – 4), as determined by the individual’s ACAT assessment.
Care Management
Ongoing monitoring and adjustments to ensure the care plan continues to meet the client’s needs, goals and preferences.
